Once for Scotland

Making non-specialist rehabilitation more accessible to people across Scotland.

Our innovative Once for Scotland programme aims to work in collaboration with early adopters (services and teams) within NHS Scotland, Health and Social Care Partnerships and the third sector, to reshape rehabilitation services for people in Scotland who are living with long-term health conditions including the ongoing impact of COVID-19.

It promotes person-centred rehabilitation that focuses on the person not the condition, where the individual, with support from friends, family and/or carers, is empowered to lead and manage their situation and remain as independent as possible.

This programme is funded by the Scottish Government and is aligned to its Once for Scotland approach to rehabilitation and recovery.

Services within health and social care have greater capacity to provide rehabilitation and supported self-management.

Teams and services interested in becoming early adopters will be invited to check themselves against an inclusion criteria. This is a set of characteristics that we have learned predict a successful outcome. See the “Get in touch” section below for more info.

A consultant from our Once for Scotland programme will get in touch and arrange an initial meeting(s) either in person or online. This discussion will aim to establish outcomes and determine what is already working in relation to reshaping services and what, if any, input may be required from the programme team. It is anticipated that many teams will benefit from a minimalist coaching type approach to support.

We will work with your team or service, free of charge, to tailor a development plan that is based on a set of options – outlined below – that best support your needs. Our team will also continue to support you throughout your journey to help you monitor, evaluate and report on your progress.

How it works

The Thistle Foundation is a leader in the health and social care sector, as well as the voluntary and independent sectors within Scotland, in inspiring and developing excellent person-centred practice among people and organisations.

Our team is comprised of experienced practitioners drawn from a variety of backgrounds including Allied Health Professionals, nursing, social work, community development and physical exercise. Practitioners combine valuable experience from their respective professional backgrounds with a core set of values and competencies, which we have developed over more than 10 years. Known to us as the Thistle Approach, it is person-centred and uses solution-focused, strengths-based 1-1 and group coaching.

We have previously supported a number of organisations, services and teams across Scotland in a similar way.

Initial engagement plans usually support teams and services to prepare by putting these criteria in place before embarking on the redesign project.
